Ariz. Admin. Code § R4-8-409 - Application for Continuing Education Approval
A. To obtain the Board's approval for a
continuing education, the provider of the continuing education shall submit to
the Board at least 45 days before teaching the continuing education:
1. A form, which is available from the Board,
containing the following information:
a.
Title of the continuing education;
b. Name and address of the continuing
education provider;
c. Name,
telephone and fax numbers of a contact person for the continuing education
provider;
d. Date, time, and place
at which the continuing education will be taught, if known;
e. Subject matter of the continuing
education;
f. Method of
instruction; and
g. Number of
continuing education hours requested; and
2. The following documents:
a. Curriculum vitae of the continuing
education instructor,
b. Objective
of the continuing education,
c.
Detailed outline of the continuing education,
d. Agenda for the continuing education
showing the hours of instruction and the subject matter taught during each
hour,
e. Method by which
participants evaluate the continuing education, and
f. Certificate of attendance that meets the
requirements at
R4-8-206(D).
B. The provider of a
continuing education that is not approved under
R4-8-408(B)
shall not advertise that the continuing education is approved by the Board
until the Board acts on an application submitted under subsection
(A).
C. The Board's approval of a
continuing education is valid for one year unless there is a change in subject
matter, instructor, or hours of instruction. At the end of one year or when
there is a change in subject matter, instructor, or hours of instruction, the
continuing education provider shall apply again for approval.
