Ariz. Admin. Code § R6-5-4907 - Withdrawal of an Application
A. An applicant may withdraw an application
at any time prior to its disposition by providing the Department with a written
request for withdrawal signed by the applicant.
B. If an applicant makes an oral request to
withdraw an application:
1. The Department
shall accept the oral request, provide the applicant with a written withdrawal
form, and request that the applicant complete the form and return it to the
Department. The Department shall inform the applicant of the consequences of
not returning the withdrawal form within 10 days of the notice date.
2. If the applicant fails to return the
completed withdrawal form, the Department shall deny the application for
failure to provide information unless the applicant rescinds the oral
withdrawal request within 10 days of the date the Department provides the
applicant a withdrawal form.
C. A withdrawal is effective as of the
application file date unless the applicant specifies a different date on the
withdrawal form.
D. An application
that has been withdrawn shall not be reinstated; an applicant who has withdrawn
an application shall reapply anew.
