Ariz. Admin. Code § R6-5-7518 - Further Administrative Appeal
A. A party may appeal an adverse decision
issued by a hearing officer to the Department's Appeals Board, as prescribed in
A.R.S. §
41-1992(C) and (D), by filing a written petition for review
with the Office of Appeals within 15 days of the mailing date of the hearing
officer's decision.
B. The petition
for review shall:
1. Be in writing,
2. Describe why the party disagrees with the
hearing officer's decision, and
3.
Be signed and dated by the party or the party's representative.
C. The party petitioning for
review shall mail a copy of the petition to all other parties.
D. The Office of Appeals shall have the
proceedings of the hearing below transcribed for the Appeals Board.
