Ariz. Admin. Code § R6-6-1010 - Recordkeeping
A. The licensee
shall maintain a record for each child which shall include the child's medical
history, dental history, educational experiences, and habilitation
services.
B. The licensee shall
obtain and provide to the Division receipts for expenditures for the child, as
required by the Division.
C. The
licensee shall maintain a personal record for the child, which may include
mementos, photos, letters, cards, report cards, school projects, art, and
toys.
D. The licensee shall keep
copies of all licenses, certificates, and correspondence in a separate file to
document compliance with sanitation, health, and environmental codes of state
and local authorities.
