Ariz. Admin. Code § R8-2-704 - Registration Denial or Revocation; Denied Compensation
A. Failure to truthfully respond to
statements set forth on the registration form may result in the denial of
registration, revocation of registration as an emergency worker, or denial of
compensation for claims arising under A.R.S §
23-1028(a).
B. Registration may be denied or revoked in
the event of the following:
1. Failure to
satisfactorily provide the information required in Section R8-2-703,
2. Health conditions that could limit the
applicant's performance as an emergency worker, or
3. Felony convictions.
