Ariz. Admin. Code § R9-10-2007 - Patient Rights
A. A licensee
shall ensure that a patient is afforded the following rights and is informed of
these rights:
1. To refuse treatment or
withdraw consent for treatment;
2.
To have patient medical records kept confidential; and
3. To be informed of proposed treatment and
associated risks, possible complications, and alternatives before pain
management services are provided.
B. A medical director shall ensure that
before an opioid is prescribed or ordered for a patient, a medical practitioner
obtains informed consent from the patient or patient's representative that
includes:
1. The patient's:
a. Name,
b. Date of birth or other patient identifier,
and
c. Condition for which an
opioid is being prescribed or ordered;
2. That an opioid is being prescribed or
ordered;
3. The potential risks,
adverse reactions, complications, and medication interactions associated with
the use of an opioid;
4. If
applicable, the potential risks, adverse outcomes, and complications associated
with the concurrent use of an opioid and a benzodiazepine or another
sedative-hypnotic medication;
5.
Alternatives to a prescribed or ordered opioid;
6. The name and signature of the individual
explaining the use of an opioid to the patient; and
7. The signature of the patient or the
patient's representative and the date signed.
