Ariz. Admin. Code § R9-10-607 - Admission
A. Before admitting
an individual as a patient, an administrator shall obtain:
1. The name of the individual's
physician;
2. Documentation that
the individual has a diagnosis by a physician that indicates that the
individual has a specific, progressive, normally irreversible disease that is
likely to cause the individual's death in six months or less; and
3. Documentation from the individual or the
individual's representative acknowledging that:
a. Hospice services include palliative care
and supportive services and are not curative, and
b. The individual or individual's
representative has received a list of services to be provided by the
hospice.
B.
At the time of admission, a physician or registered nurse shall:
1. Assess a patient's medical, social,
nutritional, and psychological needs; and
2. As applicable, obtain informed consent or
general consent.
C.
Before or at the time of admission, a personnel member qualified according to
policies and procedures shall assess the social and psychological needs of a
patient's family, if applicable.
