Ariz. Admin. Code § R9-13-102 - Student Screening Populations
a. States that the student
received an audiological evaluation from a specialist;
b. Is dated within 12 months
before the date the student would receive a hearing screening;
or
c. Includes a time period during
or after the current school year when the student is scheduled to receive
another audiological evaluation from the audiologist or specialist;
and
d. Contains the following
information:
i. The student's
name;
ii. The date the student's
audiological evaluation was performed;
iii. The type of audiological
equipment used;
iv. Whether the student has been
diagnosed as being deaf or hard of hearing and, if so, the type and degree of
hearing loss; and
v. The name of the specialist who
performed the audiological evaluation; and
3. A student who is deaf or hard
of hearing.
1. The student's
name;
2. A statement objecting to the
student receiving a hearing screening, including:
a. The school year the student
should not receive the hearing screening, or
b. Instruction the student is not
to receive a hearing screening until the parent notifies the administrator that
the student may receive a hearing screening; and
3. The parent's name, signature,
and date signed.
