Ariz. Admin. Code § R9-13-116 - Trainer Certificate of Completion Renewal
A. A training
certificate of completion may be renewed by attending and completing a
Department-approved refresher training course, either offered directly by the
Department or by a trainer authorized under this Article, during the fifth year
of certification from the date the preceding certificate was issued. Once a
refresher training course is successfully completed, the five-year cycle begins
again. If certification is not renewed within the required time period, the
individual must attend the basic certification training course (i.e., a
refresher course will not be sufficient).
B. A trainer shall submit the following to
the Department, in a Department-provided format, at least 60 calendar days
before the expiration date of the trainer's certificate of completion, which
includes the trainer's name, address, e-mail address, and telephone
number;
C. Within 30 calendar days
from the date a trainer submits a renewal certificate of completion, the
Department shall issue the trainer a certificate of completion.
