Ariz. Admin. Code § R9-22-313 - Withdrawal of Application
A.
An applicant may withdraw an application at any time before the Administration
or its designee completes an eligibility determination by making an oral or
written request for withdrawal to the Administration or its designee and
stating the reason for withdrawal.
B. If an applicant orally requests withdrawal
of the application, the Administration or its designee shall document the:
1. Date of the request,
2. Name of the applicant for whom the
withdrawal applies, and
3. Reason
for the withdrawal.
C.
An applicant may withdraw an application in writing by:
1. Completing an Administration-approved
voluntary withdrawal form; or
2.
Submitting a written, signed, and dated request to withdraw the
application.
D. The
effective date of the withdrawal is the date of the application.
E. If an applicant requests to withdraw an
application, the Administration or its designee shall:
1. Deny the application, and
2. Notify the applicant of the denial
following the notice requirements under
R9-22-307.
