Ariz. Admin. Code § R9-25-1003 - Changes Affecting Registration (Authorized by A.R.S. Sections 36-2202(A)(4) and (5), 36-2209(A)(2), 36-2212, 36-2238, and 36-2247)
A. At least 30
days before the date of a change in a certificate holder's name, the
certificate holder shall send the Department written notice of the name
change.
B. Within 30 days after the
date of receipt of a notice described in subsection (A), the Department shall
issue an amended certificate of registration that incorporates the name change
but retains the expiration date of the current certificate of
registration.
C. No later than 10
days after a certificate holder ceases to operate a ground ambulance vehicle,
the certificate holder shall send the Department written notice of the date
that the certificate holder ceased to operate the ground ambulance vehicle and
of the certificate holder's intention to relinquish the certificate of
registration for the ground ambulance vehicle as of that date.
D. Within 30 days after the date of receipt
of a notice described in subsection (C), the Department:
1. Shall:
a.
Void the certificate of registration for the ground ambulance vehicle;
and
b. Send the certificate holder
written confirmation of the voluntary relinquishment of the certificate of
registration, with an effective date that corresponds to the written notice;
and
2. If the ground
ambulance vehicle is the only ground ambulance vehicle operated by a ground
ambulance service, may revoke the certificate of necessity of the ground
ambulance service.
E. A
certificate holder shall notify the Department in writing within one working
day after a change in the certificate holder's eligibility to hold a
certificate of registration for a ground ambulance vehicle under
R9-25-1001(A)(2) or (3).
F. Upon
receiving a notification required in subsection (E), the Department:
1. Shall revoke the certificate for the
ground ambulance vehicle; and
2. If
the ground ambulance vehicle is the only ground ambulance vehicle operated by a
ground ambulance service, may revoke the certificate of necessity of the ground
ambulance service.
