Ariz. Admin. Code § R9-25-406 - Requirements for Downgrading of Certification (Authorized by A.R.S. Sections 36-2202(A)(2), (A)(3), (A)(4), and (H) and 36-2204(1) and (6))
An individual who holds current EMCT certification at a classification level higher than EMT and who is not under investigation according to A.R.S. § 36-2211 may apply for:
1. Continued
certification at a lower EMCT classification level for the remainder of the
certification period by submitting to the Department:
a. A written request containing:
i. The EMCT's name, address, email address,
telephone number, date of birth, and Social Security number;
ii. The lower EMCT classification level
requested;
iii. Attestation that
the applicant has not committed an act or engaged in conduct that would warrant
revocation of a certificate under A.R.S. §
36-2211;
iv. Attestation that all information submitted is true
and accurate; and
v. The applicant's
signature or electronic signature and date of signature; and
b. Either:
i. A written statement from the EMCT's
administrative medical director attesting that the EMCT is able to perform at
the lower EMCT classification level requested; or
ii. If applying for continued certification
as an EMT, an Arizona EMT refresher certificate of completion or an Arizona EMT
refresher challenge examination certificate of completion signed by the
training program director designated for the Arizona EMT refresher course; or
2.
Recertification at a lower EMCT classification level according to
R9-25-404.
