Ariz. Admin. Code § R9-6-601 - Reporting Requirements
A physician or an authorized designee shall submit a written or electronic report to the Department for each individual exposed who receive post-exposure rabies prophylaxis that includes:
1. Name, age, address, and telephone number
of the individual exposed;
2. Date
of report;
3. Reporting institution
or physician;
4. Date of
exposure;
5. Body part
exposed;
6. Type of exposure: Bite
or saliva contact (non-bite);
7.
Species of animal;
8. Animal
disposition: quarantined, euthanized, died, unable to locate;
9. Animal rabies test results, if any:
positive or negative;
10. Treatment
regimen; and
11. Date treatment was
initiated.
