Ariz. Admin. Code § R9-7-516 - Records of Receipt and Transfer of Sealed Sources
A. A licensee shall maintain records that
show each receipt and transfer of a sealed source or device that uses DU for
shielding and retain each record for three years after it is made.
B. The records shall contain separate entries
for each transaction, including the date, name of the individual making the
record, radionuclide, number of Becquerels (curies) or mass (for DU), and
manufacturer, model, and serial number of each sealed source or device, as
applicable.
