Ariz. Admin. Code § R9-9A-105 - Application for License Renewal
A. A
license is valid for two years from the date of issuance or renewal as
specified in A.R.S. §
36-851.01(C).
B. At least 30 calendar days before the
expiration date indicated on a procurement organization's license, a licensee
shall submit to the Department an application for renewal that contains:
1. The following, in a Department-provided
format:
a. The licensee's name, mailing
address, email address, and telephone number;
b. The procurement organization's license
number;
c. Whether the applicant
agrees to allow the Department to submit supplemental requests for information
under R9-9A-109; and
d. The
licensee's signature and the date signed;
2. If applicable, documentation that the
procurement organization is accredited; and
3. An application fee of
$2,000.
C. The Department
shall renew or deny renewal of a license as specified in R9-9A-109.
