Cal. Code Regs. Tit. 16, § 1678 - Application and Renewal
(a)
Applications for a fictitious name permit (Application for a Fictitious Name
Permit OMB.5 Rev.01/92) shall include the following information:
(1) the name style;
(2) if applicable, proof that the place of
professional practice of the applicant or applicants is wholly owned or leased
by the same; and
(3) if applicable,
proof the professional practice of the applicant or applicants is wholly owned
or entirely controlled by the same.
(b) The application shall be signed by a
licensed person who is a member of the group or clinic or by an officer of the
professional corporation, as the case may be, requesting the use of such name.
A copy of the approved Articles of Incorporation shall be submitted with the
application for all professional corporations.
(c) The executive director of the Board shall
issue a fictitious name permit or refuse to approve the application and shall
notify the applicant of the reasons therefor within the processing time limits
set forth in Section
1691.
(d) The applicant shall include the initial
permit fee as set forth in Section
1690; additionally, fictitious
name permits shall expire on December 31 of each year and shall be renewed upon
payment of the fee set forth in Section
1690; processing time limits for
renewals are set forth in Section
1691.
