Cal. Code Regs. Tit. 25, § 5548 - Statement of Error or Erasure
(a) A person, who has in error signed,
erased, lined through, altered a name or signature that denotes ownership,
shall complete and submit to the department with the application for
registration, a statement signed under penalty of perjury, which contains the
following information:
(1) The unit
identification and
(2) A statement
indicating the line number and the name of the form on which the error occurred
and certifying to the reason for the error or erasure.
