Cal. Code Regs. Tit. 3, § 1180.25 - Requirements for Liability Insurance or Surety Bond for Transporters of Inedible Kitchen Grease

(a) Proof of insurance shall include a letter or certification form, such as ACORD Form 25 Certificate of Liability Insurance (2001/08) that indicates that the insurance policy meets the requirements of section 19310 of the Food and Agricultural Code.
(b) Proof of surety bond shall be on a form that has been approved as to conformity with applicable law by the California Attorney General's Office and that indicates liability coverage of the surety bond meets the requirements of section 19310 of the Food and Agricultural Code.
(c) Proof of insurance or surety bond shall be submitted together with the transporter's application for registration and annually thereafter when renewing registration with the Department.
(d) The person or company named in the proof of insurance or surety bond shall be the same person or company named in a newly submitted or completed MPES Form 79-012A(1) (Rev. 08/11) Personal Use Inedible Kitchen Grease (Used Cooking Oil) Transporter Registration Application or MPES Form 79-012A(2) (Rev. 08/11) Commercial Use Inedible Kitchen Grease Transporter Registration Application that is in the possession of the Department.
(e) All vehicles covered by the insurance policy or surety bond certificate shall be the same vehicles listed in the completed MPES Form 79-012A(1) (Rev. 08/11) Personal Use Inedible Kitchen Grease (Used Cooking Oil) Transporter Registration Application or MPES Form 79-012A(2) (Rev. 08/11) Commercial Use Inedible Kitchen Grease Transporter Registration Application that is in the possession of the Department.
