Fla. Admin. Code Ann. R. 28-106.402 - Contents of Request for Mediation
The request for mediation shall contain:
(1) The name, address, any e-mail address,
and telephone number of the party requesting mediation and of that party's
representative, if any; and
(2) A
statement of the preliminary agency action.
