Fla. Admin. Code Ann. R. 40A-3.411 - Completion Reports
(1) Within thirty
(30) days after the expiration of a permit, a well completion report shall be
filed with the District on the forms provided by the District and in accordance
with the instructions provided thereon.
(2) If no work is performed or if the well is
not completed, a report shall be filed within thirty (30) days of the
expiration of the permit stating that no well construction was performed under
the permit or outlining the status of the incomplete well.

shall be used to report the completion of construction, repair, or abandonment
of any well within District jurisdiction. A copy can be obtained from district
offices upon request.
(4) The water
well contractor shall provide the well owner a copy of the well completion
report.
(5) The information filed
as a part of the completion report shall be truthful and accurate in providing
the specifications and characteristics of the constructed well. (See subsection
