Fla. Admin. Code Ann. R. 58D-1.003 - Eligibility
(1) To be eligible for model day care
services, an individual must be at least 18 years of age and have a diagnosis
of Alzheimer's disease or a related memory disorder.
(2) To be eligible to receive all other
services funded under the Alzheimer's disease Initiative that are included in
the provider's contract with the department or the area agency on aging, an
individual must be 18 years of age or older and have a diagnosis of Alzheimer's
disease or related memory disorder, or be suspected of having Alzheimer's
disease or a related memory disorder.
(3) The caregivers of individuals receiving
services under the Alzheimer's Disease Initiative are eligible to receive
training and related support services to assist them in caring for the person
with ADRD.
