Fla. Admin. Code Ann. R. 59G-4.105 - Dialysis Services
(1) This rule
applies to all providers of dialysis services who are enrolled in or registered
with the Florida Medicaid program.
(2) All providers of dialysis services must
comply with the Florida Medicaid Dialysis Services Coverage Policy, December
2015, incorporated by reference. The policy is available from the Medicaid
fiscal agent's website at http://portal.flmmis.com/FLPublic,
