Fla. Admin. Code Ann. R. 59G-4.210 - Visual Care Services

(1) This rule applies to any person or entity prescribing or reviewing a request for visual care services and to all providers of visual care services who are enrolled in or registered with the Florida Medicaid program.

(2) All persons or entities described in subsection (1), must be in compliance with the provisions of the Florida Medicaid Visual Care Services Coverage Policy, June 2016, incorporated by reference.
