(1) Any person desiring to apply for an examination or re-examination and licensure shall submit a completed Form DBPR LA 1, Application for Licensure: Examination or Re-Examination, effective November 2022, adopted and incorporated by reference,

(2) Any person desiring to apply for licensure by endorsement shall submit a completed Form DBPR LA 3, Application for Licensure: Endorsement, effective November 2022, adopted and incorporated by reference,
