Fla. Admin. Code Ann. R. 69A-37.0336 - Application Process Certificate of Compliance
(1) An applicant shall make application for a
certificate of compliance on form DFS-K4-1016, effective January 1, 2009,
adopted and incorporated by reference in subsection
69A-37.039(2),
F.A.C. The form can be obtained where indicated in subsection
69A-37.039(1),
F.A.C. The applicant is encouraged to submit the application
electronically.
(2) Within 30 days
of receipt of the application, the Bureau shall notify the applicant of the
actions necessary for the application to be deemed complete. The notification
shall be in writing and provided either electronically or by U.S.
Mail.
(3) The application shall be
deemed complete when all of the information requested in the application has
been provided, the application blanks are all filled in, the results of the
criminal history background check have been provided, the required fees are
paid, and when the applicant has successfully completed the examination process
as prescribed by the Bureau.
(4)
The application shall be granted or denied within 90 days of the date the
application is deemed complete. The applicant shall be notified of the decision
in writing, which shall include a notice of rights and election of
proceedings.
