Fla. Admin. Code Ann. R. 69K-5.004 - Procedures for Licensing a New Cemetery
(1) Any corporation, partnership or limited
liability company wishing to establish a cemetery should review the
requirements of the Florida Funeral, Cemetery, and Consumer Services Act
(chapter 497, F.S.) and applicable rules and regulations before filing a formal
application for permission to start a cemetery in the State of Florida. The
following should then be completed and submitted to the Department:
(a) Application To Organize A New Cemetery
Company, Form DFS-CEMN, adopted in rule
69K-1.001, F.A.C., and a
non-refundable fee in the amount of $5,005;
(b) Application to Transact Cemetery
Business, Form DFS-CEM, and a license fee of $255 to $4,905 depending on the
applicant's most current annual gross sales, refundable if license is not
issued;
(c) Current Financial
Statement of each proponent, Form DFS-F-32, adopted in rule
69K-1.001,
F.A.C.
(2) An application
may be submitted to the Department by mail to the Division of Funeral, Cemetery
and Consumer Services, Revenue Processing, P.O. Box 6100, Tallahassee, Florida
32399-6100. All forms herein are available by mail from The Department of
Financial Services, 200 East Gaines Street, Tallahassee, Florida 32399-0361 or
