Fla. Admin. Code Ann. R. 69L-10.0065 - Filing; Facsimile Transmissions
(1)
All claims, notices, legal documents, forms, affidavits, reimbursement
requests, and documentation shall be submitted by mail or in person at the
Tallahassee office of the SDTF. The filing date shall be the date
received.
(2) Documentation which
is requested by the SDTF subsequent to receipt of the notice of claim and proof
of claim may be submitted by facsimile transmission, provided there is a prior
agreement between the sender and the SDTF regarding such specific transmission.
The Fund will make the determination based on the urgency and volume of the
transmission. All such facsimile transmissions shall be completed between the
hours of 8:00 a.m. and 5:00 p.m. (EST) or (EDT).
(3) Any document which is required to be
submitted by mail or in person may not be submitted by facsimile transmission
and will not be accepted or effective until proper service is completed as set
out in subsection (1).
