Fla. Admin. Code Ann. R. 69O-161.003 - Use of Industrial Life Insurance Claim Form

All companies writing industrial life insurance shall use the Uniform Life Insurance Claim Form (OIR Form 4-371) (2/94) which is hereby incorporated and made part of this rule chapter by reference.
