The written notice to qualify as a qualified limited service affiliate shall be filed on Form OFR-U-135, Written Notice to Qualify as a Qualified Limited Service Affiliate in the State of Florida, effective 02/2023, herein incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-15201, or at the OFR's website, http://www.flofr.gov.
