Haw. Code R. § 11-110.1-36 - Fees
(a) The department shall not issue an
original license or renew a license without first collecting the appropriate
fee:

| (3) License renewal fee: | |

| (4) Restoration fee: | $20 |
(b) All fees shall be collected by the
department at the time of application for initial licensure, license renewal,
and restoration of license.
(c)
Requests for license renewal and the renewal fees must be received by the
department by January 31 of each odd-numbered year starting in the year 2003.
The failure, neglect, or refusal of any person holding such license to request
renewal or to pay the renewal fee after thirty days of delinquency shall
constitute a forfeiture of the person's license; provided that a forfeited
license may be restored upon written application and payment of all delinquent
fees. If a license is forfeited for one year and less than two years, the
department may request that the applicant submit additional information and
evidence satisfactory to the department, including passing of an examination,
or showing that the applicant remains fit to practice as a clinical laboratory
personnel, or both before the license is restored. A license that has been
forfeited for two years or more shall not be restored and a new application for
license shall be required.
(d) A
fee shall be paid for the restoration of a license suspended pursuant to
chapter 436C, HRS, or pursuant to chapter
576D, HRS.
