Haw. Code R. § 11-113-19.1 - Approval of laboratories
(a) Any laboratory
which is certified by the Substance Abuse and Mental Health Service
Administration (SAMHSA) may apply to the department for approval to perform
substance abuse tests on samples from the state of Hawaii.
(b) A laboratory seeking approval shall
submit to the department a duly completed application form.
(c) Each approval shall expire within twelve
months from the date of approval.
(d) Approved laboratories must use SAMHSA
approved methods for testing samples from Hawaii.
(e) Methods for testing non-SAMHSA analytes
must be approved by the director.
(f) Laboratory approval will be revoked if
any one of the provisions of subsections (a) through (e) is not met.
(g) The director shall keep a current listing
of all approved laboratories which perform substance abuse tests on samples
from Hawaii. This listing may be sent to any third party upon written
request.
