Haw. Code R. § 11-175-47 - Right to choose a primary provider of service

Psychiatric facilities and residential treatment facilities shall establish policies and procedures for the consumer's right to choose a primary provider of service, or request a change in primary-provider. These policies and procedures shall include:

(1) How a consumer is assigned a primary provider of service;

(2) How the consumer may request a change of primary provider; and

(3) Limitation of this right only when:

(A) The agreed upon treatment plan does not include a primary provider; or

(B) The person chosen by a consumer is not available or does not agree.
