Haw. Code R. § 11-175-61 - Right to coordinated services
(a) When a consumer
is admitted to a psychiatric facility and the consumer does not have a
previously assigned case manager, the facility administrator or program
administrator shall ensure that the consumer is offered case management
services. Case management services shall include discharge planning, planning
for services which may be needed subsequent to discharge, and follow up after
discharge for assistance in obtaining such services.
(b) A consumer admitted to a residential
treatment facility shall be offered the services of a staff person who shall:
(1) Serve as the consumer's case manager or,
if the consumer has a case manager assigned by the service area center, act as
a liaison with the case manager;
(2) Assist the consumer, as necessary, in
obtaining other mental health and substance abuse services and in gaining
access to other appropriate community services; and
(3) Serve as an advocate for the consumer
within and outside the program.
