Haw. Code R. § 11-177.1-39 - Revocation
(a) A certificate may be revoked for any of
the reasons specified in section 11-177.1-34. The letter of revocation shall:
(1) Be in writing; and
(2) State the reason for the revocation and
the effective date of revocation.
