Haw. Code R. § 11-186-22 - Filing of application
The applicant for a certificate of need shall file an application with the agency The application shall be filed on the form prescribed and furnished by the agency The agency shall not accept for filing any application that is not submitted on the correct form If the agency inadvertently accepts for filing an application that is not submitted on the correct form, the agency shall return the application to the applicant and require the applicant to submit the application on the correct form.
