Haw. Code R. § 11-219-7.75 - Renewal of disabled paid parking exemption permits
(a) A
disabled paid parking exemption permit may be renewed up to sixty days prior to
the expiration of the permit.
(b)
To renew a disabled paid parking exemption permit, a completed renewal
application form and a legible copy of a valid driver's license shall be
submitted to the issuing agency by mail. The issuing agency shall process the
form and issue a new disabled paid parking exemption permit and identification
card to the applicant by mail. The certificate of disability section of the
form shall be completed by a licensed practicing physician or an advanced
practice registered nurse and shall be valid for no more than one-hundred
eighty days from the date of certification.
(c) If a person with a disability has been
issued one set of special license plates and one disabled paid parking
exemption Permit, both expire at the same time.
