Haw. Code R. § 11-33-3 - Hawaii drug formulary; establishment, revision, and information
(a) The "Hawaii Drug Formulary of Equivalent
Drug Products" dated May, 1981, appended to this chapter is hereby established
and made a part of this section. The Hawaii drug formulary shall be reviewed
annually and may be revised as often as necessary to conform to changes made in
"A" coded drugs.
(b) All comments
and requests for specific information regarding drug products listed in the
Hawaii drug formulary shall be directed to the following:
