Haw. Code R. § 11-45-102 - Utilization logs

Each licensee shall maintain current logs which shall be kept available for inspection by the department for three years, showing for each x-ray system the following information:

(1) A unique identification, such as a serial number, of each x-ray system;

(2) The identity of the radiographer to whom assigned;

(3) Locations where used and dates of use; and

(4) The date(s) each x-ray system is removed from storage and returned to storage.
