Haw. Code R. § 11-504-11 - Renewal of certification
(a) Any application
for renewal of certification shall contain all the information requested by
these rules without reference to previously submitted material.
(b) Evidence of successful completion of an
accredited annual refresher course, specific to each discipline, shall be
provided with the renewal application.
