Haw. Code R. § 11-504-6 - Certification application
Applications shall be made on forms provided by the director. The department may require further information to better determine whether an application should be approved or denied. If the department requests further information from an applicant, and does not receive that information within sixty days, the application shall be considered incomplete and shall be denied.
