Haw. Code R. § 11-800-12 - Reporting changes
The case management agency or home shall immediately report to the department changes that may affect the case management agency's or home's ability to comply with the applicable requirements of this chapter. Changes to be reported include but are not limited to changes:
(1) That may pose a risk to the life, health,
safety, or welfare of the client;
(2) In the criminal history record and adult
protective service perpetrator history of the operator or any employee of the
case management agency, or the primary caregiver, substitute caregiver, or
other adults in the home, except for clients receiving care;
(3) In the organizational structure of the
case management agency;
(4) In the
household composition or structure of the home; and
(5) In the service delivery site.
