Haw. Code R. § 11-850-38 - Reports
(a) A dispensary licensee shall submit
quarterly reports on January 31, April 30, July 31, and October 31. If the due
date for submitting a quarterly report falls on a Saturday, Sunday, or State
holiday, the report will be on time if it is submitted on the next day that is
not a Saturday, Sunday, or State holiday. Reports shall be submitted on a form
and in a manner prescribed by the department.
(b) Reports shall include:
(1) Records of entry and exit for all
individuals who entered a dispensary facility;
(2) Amounts by category of cannabis produced
and manufactured cannabis products manufactured, purchased from other
dispensary, and offered for retail sale;
(3) Amounts by category of cannabis and
manufactured cannabis products sold at retail dispensing locations and sold to
another dispensary;
(4) A list of
all cannabis, manufactured cannabis products, or unusable cannabis materials
that have been destroyed or will be destroyed;
(5) A summary financial statement;
(6) Laboratory results of all analyses
conducted;
(7) Description of any
breach or halt in its security system and tracking system;
(8) An updated list of employees;
and
(9) Any other information
requested by the department.
