Haw. Code R. § 11-96-26 - Occupational therapy services
(a) Occupational
therapy services which utilize purposeful activity shall be made available in
each center as necessary, and be provided under a physician's order, and shall
include, but not be limited to, the following:
(1) Assessment of the client and formation of
an individual plan of care in conjunction with the multidisciplinary team;
and
(2) Progress notes written at
least quarterly in the clients' health record and signed by the occupational
therapist.
(b) The
occupational therapy services shall be under the supervision of an occupational
therapist.
(1) An occupational therapy
assistant shall work only under the supervision of an occupational therapist;
and
(2) An occupational therapy
aide shall work only under the immediate supervision of an occupational
therapist or an occupational therapy assistant.
(c) Adequate space shall be provided for
occupational therapy services and for storage of equipment.
