Haw. Code R. § 11-99-4 - Active treatment program
(a) A plan of
treatment shall be developed and implemented for each resident in order to help
the residents function at their greatest physical, intellectual, social,
emotional, and vocational level.
(b) A provisional treatment plan shall be
developed within one week of admission by a qualified mental retardation
professional. A definitive treatment plan shall be developed within one month
and shall be based on evaluation by an interdisciplinary team including the
qualified mental retardation professional and at least a physician, a social
worker, a psychologist, and a nurse.
(c) The plan shall be reviewed at least
quarterly by a qualified mental retardation professional member of the
interdisciplinary team who is designated as the coordinator for the resident's
plan of care.
(d) The facility
staff shall participate in the quarterly review of the plan.
(e) The plan shall include all aspects of the
resident's program including services provided outside the facility.
(f) All reviews and modifications of the plan
shall be documented in the resident's record which is retained at the
facility.
