Haw. Code R. § 12-15-53 - Hospital services
(a) When
hospitalization is required for further treatment of an injured employee, that
employee shall have a free choice of a licensed hospital on the island where
the injury occurred. If the employee is in critical condition or unable to
express a choice or requires service from a hospital not located on the island
where the injury occurred, then the attending physician may designate the
hospital to which the injured employee will be taken.
(b) Hospital charges for an injured employee
shall be limited to the lowest room charge for the nature of the injury at the
hospital where confined, except if the nature of the injury requires private
care, intensive care, or isolation, as determined by the attending physician,
in which case the prevailing private rates may be charged.
(c) Where an injured employee is treated in
the emergency facility of a hospital, the allowable hospital charge for the use
of the emergency room shall be the established emergency room charge for that
particular hospital.
(d) All
hospital charges shall be itemized when a bill is submitted.
