Haw. Code R. § 13-168-11 - Registration of existing wells
(a) Within one year
from the effective date of these rules, the owner or operator of any well in
existence on the effective date of these rules shall register the well with the
commission on forms provided by the commission. The owner or operator shall
disclose the location of such well and all other facts or information related
to its geology, hydrology, and construction. Registration shall include, but
not limited to, such information as water use permit number, if any; location
and dimensions of the well; state-assigned well number; depths and diameters of
drilled hole and casing; range of water level and salinity; pumping test
results, if any; installed pump description and operating capacity; method of
measuring water usage; method of construction; and well driller, if
known.
(b) The commission may deny
the issuance of a water use permit as provided for under chapter 13-171 until
such time as the applicant registers all wells which the applicant owns or
operates.
