Haw. Code R. § 13-251-5 - Application for operator permit
(a) Every
application for an operator permit shall be accompanied by the proper fee and
payment of such fee shall entitle the applicant to not more than three attempts
to pass any examination within a period of six months from the date of
application.
(b) Every application
shall include the full name, aliases, date of birth, sex, and resident address
of the applicant and briefly describe the applicant, and the applicant's
experience related to the vessel, surfboard, or sailboard the applicant wishes
to use or operate. The application shall state whether the applicant has been
issued a permit and, if so, when and whether the permit has ever been suspended
or revoked, or whether an application has ever been refused and, if so, the
date and reason for the suspension, revocation, or refusal. Two recent
photographs shall be attached to the application.
