Haw. Code R. § 13-251-7 - Operator permits issued to catamaran captains, canoe captains, canoe second captains, surfboard instructors, sailboard instructors and commercial motorboat operators
(a) An operator
permit when issued by the department shall bear a distinguishing number
assigned to the permit, the full name, date of birth, residence address, and
brief description of the permittee, and a space upon which the permittee shall
sign in ink the permittee's name immediately upon receipt of the permit. No
permit shall be valid until it has been so signed by the permittee.
(b) Every operator permit shall bear the
permittee's photograph showing the permittee's facial features, which shall be
furnished by the permittee.
