Haw. Code R. § 14-51-26 - Rejection and cancellation; notification

(a) An application for benefit election shall be rejected or canceled if:
(1) The form is incomplete;
(2) An entry in the form is inconsistent with the plan or this chapter; or
(3) The applicant is not eligible under a component plan.
(b) If an application is rejected or canceled, the applicant shall be notified, in writing, of the action and the reasons therefore aid shall have appeal rights as provided under section 14-51-51.
