Haw. Code R. § 14-59-5 - Changes to enrollment form
(a) It shall be the
responsibility of the participant to notify the investment provider in writing
of any change in an entry on the enrollment form.
(b) If a change would be inconsistent with
the plan or this chapter, the investment provider shall promptly notify the
participant in writing that the change cannot be made and the reason why it
cannot be made.
